We are seeking a highly organized Case Manager to join our team in Atlanta, GA. The successful candidate will be responsible for managing a caseload of clients, coordinating services, and ensuring that clients receive the support they need to achieve their goals.
Responsibilities: Manage a caseload of clients and provide ongoing support
Coordinate services and resources to meet the needs of clients
Develop and implement individualized service plans
Monitor client progress and adjust service plans as needed
Collaborate with other service providers and community resources
Maintain accurate and up-to-date client records
Provide crisis intervention and support as needed
Requirements: Bachelor's degree in social work, psychology, or related field
Minimum of 2 years of experience in case management
Excellent organizational and time management skills
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Knowledge of community resources and services
Ability to maintain confidentiality and adhere to ethical standards
