Case Manager (CTCM) – Community Reentry Assistance Network (CRAN) – Bronx, NY
Join EAC Network as a full‑time Case Manager (CTCM) supporting behavioral health and justice‑involved clients in the Bronx Community Reentry Assistance Network. Some travel to Riker’s Island is required.
Wage & Schedule
Wage: $28.93 per hour
Schedule: Monday‑Friday, 9 AM–5 PM (35‑hr week)
Location: 1020 Grand Concourse, Bronx, NY
Core Purpose
The Community Transitions Case Management Team provides screening, treatment planning, case management, and court reporting for eligible substance‑abusing and/or mentally ill clients.
Key Responsibilities
Collaborate with Rikers Island discharge planners and the court system; conduct in‑person, video, and phone screenings for incarcerated clients before release.
Perform comprehensive screening and, with supervision, refer clients to mental health, drug abuse, or alcoholism facilities, arranging transportation when necessary.
Assist clients in applying for and receiving public and private benefits, linking them to appropriate community providers.
Maintain comprehensive case documentation (progress notes, etc.) for supervisory review.
Manage a caseload, monitoring client participation through appointments, progress reports, and conferences with treatment personnel.
Prepare and deliver written and oral status reports to the criminal justice system regarding client progress.
Investigate and recommend supportive services for clients.
Conduct drug testing as needed and perform timely visits to treatment and correctional facilities.
Perform all other duties as directed by supervision and EAC Network administration.
Requirements
Bachelor’s degree in Forensic Psychology, Psychology, Criminal Justice, or behavioral science.
Strong oral and written communication skills; ability to work cooperatively with treatment and criminal justice systems.
Proficiency with computer applications, including spreadsheets, Microsoft Office, calendars, email, and database software.
