Job Description
This position is responsible for providing the community relationship development with health, human, and social service community partners to best encompass the community‑centric needs of that position’s main site. The position serves as a resource representative for citizens in each district area of the community to help navigate needs and issues such as food insecurity, homelessness and emergency housing, utility and rental assistance, and other targeted social needs emerging currently and in the future . Each community has a different dynamic, and sources of need, and the navigators identify and collaborate with the community partners onsite and within the community at large through active communication, decision‑making, and problem‑solving. Duties may involve participating in a support partner coordination, conducting an assessment of residents’ needs, and helping people obtain direction for the services they need. This position will also provide the key resource and assistance necessary at the Community Resource Centers for current health partners and the community area served onsite.
Essential Duties
Coordinat ing services with community resources and agencies to address resident needs such as food insecurity, housing, utilities, and other emerging social issues.
Maintain ing detailed and timely resident records, including case notes, progress updates, and key service activities.
Monitor ing participant progress and follow up on referrals through virtual or in‑person outreach to identify resource needs, connect residents to appropriate services, and evaluate the effectiveness of interventions to ensure needs are met.
Collabora ting with partners and programs to expand mobile senior services and community access initiatives, maintaining up‑to‑date knowledge of community resources to support effective referrals.
Communicat ing and advocate with internal and external partners to identify and resolve community needs.
Deliver ing education and outreach to engage residents, organizations, and stakeholders through One Stop for Help programs and related initiatives.
Uphold ing ethical standards, cultural competency, and professional boundaries to provide excellent customer service and maintain sensitive interactions.
Support ing part‑time Warming Station attendants and assist with rotating station support, including assistance with delivery and offloading supplies. (e.g., cots, water, snacks, cot covers, blankets) using county vehicles
Collect ing intake files and engage with residents on‑site to ensure needs are met and documentation is complete.
Conduct ing site visits to encampments where initial contact has been established by a Police Officer, Community Representative, or local residents and safely engages with individuals or groups to share resources and offer support.
Public Speaking Duties
Educat ing and provid ing information about programs, services , and opportunities to the public at community events, resource and job fairs, and other forums, as needed.
Developing and engaging in relationship building through outreach with groups, agencies, organizations, and resident‑related interactions.
Working onsite at partner locations and gauging resident needs with One Stop for Help operational assistance and resource sharing.
Other Job Requirements / Information
Ability to work and be flexible to a variety of hours including some seasonal rotational weekends, daytime, evenings, and holidays.
Have knowledge of case management principles; applicable laws, rules, and regulations; counseling principles and practices; available resources to clients; and screening and assessment principles and practices.
Have skills in providing case management; applying applicable laws, rules, and regulations; screening and assessing clients; resolving complex and sensitive issues; preparing and maintaining records and reports; communication and interpersonal skills as applied to interaction with coworkers, supervisors, the general public, etc. sufficient to exchange or convey information and to receive work direction.
Grade
The grade for this position is B241. Salary will be dependent on education and experience.
Effective 4/11/2023, this position is eligible for the Employee Referral Program.
Minimum Qualifications
Bachelor’s Degree in Social Work or related field
One year of experience in case management
Or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above
Valid driver's license
CPR (within six months of employment)
First Aid (within six months of employment)
Preferred Qualifications
Bachelor's Degree in Social Services, Social Work, Sociology, Community Development, Public Health, Human Services, Psychology, Public Administration or Education (Adult, Special, or Early Childhood)
One year of experience in case management principles, applicable laws, rules, and regulations
