Case Manager/Community Health Worker

What You'll Do:

• Manage a caseload and conduct outreach (telephonic & community-based)
• Complete assessments in-home or in the community
• Coordinate medical services and referrals
• Assist participants in accessing social services and making appointments
• Conduct off-site visits to homes, hospitals, and community agencies
• Collaborate with interdisciplinary teams and contribute to program improvements
• Maintain accurate documentation in EMR
• Participate in outreach and marketing activities

What you Bring:

• Bachelor's degree or equivalent experience (Community Health Worker certification accepted)
• Experience with adults with disabilities, chronic illness, behavioral health, or substance use
• Strong knowledge of Rhode Island community resources
• Excellent communication and cultural sensitivity
• Ability to work independently and as part of a team
• CPR/BLS certification (American Heart Association or Red Cross)
• Commitment to confidentiality and HIPAA compliance
