Overview
The Clean and Sober Shelter provides a safe, structured, and closely monitored environment for unsheltered single adults experiencing homelessness. With a recovery-focused orientation, the shelter offers secure sleeping spaces, nutritious meals, and comprehensive case management to support participants in achieving housing stability. Under the supervision of the Program Manager, the main goal of the Case Manager is to address the needs of shelter participants and cultivate a transformational community to support their transition to drug treatment or stable/permanent housing and self-sufficiency.

Duties and Responsibilities

Manage an assigned caseload of up to 25 program participants.

Conduct assessments and intake interviews to understand client needs, situations, and risks; gather relevant background information.

Create individualized transition plans outlining goals and interventions tailored to each client.

Establish and maintain relationships with partner organizations and referring agencies.

Monitor client progress and reassess needs to keep plans relevant and effective.

Educate and empower clients with knowledge and skills to improve their situations and foster self-sufficiency.

Respond to and manage crisis situations encountered by clients.

Maintain professional, ethical boundaries with staff and clients.

Be punctual, flexible, and reliable.

Keep accurate client records using The Salvation Army Social Services Database.

Attend all required staff, Corps, and Divisional meetings and training sessions.

Facilitate small group instruction as scheduled.

Participate in community-based meetings as needed.

Identify resources, programs, and services beneficial to clients and refer accordingly.

Coordinate with service providers to meet clients' holistic needs.

Perform other duties as assigned by the Program Director or Manager.

Assist in planning and executing events such as graduations, celebrations, and group activities.

Transport clients to appointments or services as needed.

Perform other related duties as required.

Education and Experience

Minimum two years of experience providing social services to marginalized populations, especially adults in recovery, reentry, or drug treatment.

At least one year of experience in case management, peer mentorship, group facilitation, or related support roles.

Experience working with issues of substance abuse, mental illness, and barriers to self-sufficiency.

High school diploma or equivalent required.

Preferred: Associate’s Degree, Community Health Worker Certificate, or Certified Alcohol and Drug Counselor.

Valid CA Driver's License required.

Spanish bilingual skills preferred.

First Aid and CPR certification to be obtained within the first seven days of employment (provided by company).

Tuberculosis clearance within first fourteen days of employment (provided by company).

Skills and Abilities

Strong verbal and written communication skills.

Ability to express empathy and foster collaboration with clients.

Excellent organizational skills and ability to work independently.

Effective time management skills.

Proficiency in electronic communication and Microsoft Word & Excel.

Knowledge of San Francisco social services, government resources, healthcare, mental health, and housing services.

Understanding of co-occurring disorders and trauma-informed care.

Culturally competent and capable of working with a diverse population.

Bilingual in Spanish preferred.

Physical Requirements
Ability to lift up to 25 lbs. (e.g., file boxes).

Equipment Used
Modern office equipment and relevant software.
