The Qualified Mental Health Professional will provide Targeted Case Management services to qualifying Center clients. Care Coordinator will be trained in and deliver family-oriented, individual and/or group psychosocial rehabilitative services, skills training, medication training, and/or case management services according to best practice standards set forth in the Texas Recovery & Resilience Guidelines. The Targeted Case Management Care Coordinator will provide services in a variety of community settings.
Essential Duties and Responsibilities
• Complete training and certification in all evidence-based methodologies plus all versions of Child and Adolescent Needs and Strengths (CANS) and Adult Needs and Strengths Assessment (ANSA), not limited to but including Wraparound, when applicable.
• Maintain all certifications valid, renewing training and re-certification annually or as needed.
• Become certified as Qualified Mental Health Professional-Clinical Setting (QMHP-CS) and participate fully in credentialing process with El Paso Child Guidance Center (EPCGC).
• Maintain caseload of TCM clients, completing also associated administrative and documentation tasks.
• Conduct CANS/ANSA assessments with clients every 90 days for children/adolescents and 180 days for adults, ensuring timely completion of all assessment and paperwork.
• Ensure all daily documentation is completed and all required databases entered in a timely and efficient manner of twenty-four business hours after service delivery.
• Provide crisis intervention and safety planning for children, adolescents and their families as needed.
• Develop and document treatment plans, including goals and objectives, on a timely basis or no later than every 90 days for children/adolescents and 180 days for adults.
• Provide skills training, case management, psychosocial rehabilitation and/or medication management services, in alignment with the client's treatment plan and authorized level of care units and insurance authorizations based on client needs.
• Maintain client confidentiality always and participate in interdisciplinary case staffing meetings, training, and supervision as appropriate.
• Maintains compliance with all policies and procedures of the EPCGC, including compliance with Counsel on Accreditation (COA) standards and all other regulatory requirements.
