Case Manager POSITION TITLE: Case Manager
DEPARTMENT: Shelters
REPORTS TO: Supervising Case Manager
FLSA: Full-time, Non-Exempt
PAY RANGE: $22.00/hr – $26.00/hr, commensurate with experience
Position Summary The Case Manager is responsible for creating a positive and empathetic environment while assisting residents in accessing housing and services. Through one‑on‑one case management and coordination with partner agencies, the Case Manager guides clients toward securing housing, securing medical insurance, increasing income, and achieving self‑sufficiency.
Essential Functions Perform intakes, interviews, and assessments and refer clients to community resources.
Work with clients to identify and overcome barriers to securing housing and increasing income.
Provide encouragement and direction to clients.
Assist clients in obtaining and completing necessary documents for housing, benefits, or income increases.
Maintain prompt documentation using HMIS, following agency standards.
Assess client needs and coordinate services, including medical, substance abuse, therapeutic, and social services.
Connect clients with partner agencies and services.
Assess clients for employment, Social Security, and disability eligibility.
Assist with benefits enrollment.
Coordinate transportation for appointments.
Help clients acquire necessary identification and proof of status.
Assist residents in securing permanent housing.
Maintain productive case management relationships.
Meet with clients weekly to review and support individualized plans.
Address unanticipated client/resident needs or issues.
Participate in a multidisciplinary team to develop client action plans.
Other Responsibilities Mandated reporting.
Collaborate with team members.
Participate in staff training, meetings, and development activities.
Perform additional duties as directed by management.
Assist with caseload assignments, client interactions, interventions, and documentation.
Assist with provider coordination and reporting/auditing.
Qualifications Candidates must demonstrate adherence to policies and procedures, and contribute to a positive work environment.
Education & Experience Bachelor’s degree in healthcare, psychology, sociology, social work, public administration, or related field AND 1 year of applicable experience; OR
Associate’s degree in a related field AND 2 years of applicable experience; OR
High school diploma or GED AND minimum of 2 years of lived experience plus 2 years of practical job experience.
Certifications CPR, First Aid, and NARCAN certification required.
Other Requirements In-person and telephone communication skills required.
Proficiency with Microsoft Office (Outlook, Word) required.
Strong written and verbal communication skills required.
Ability to adhere to policies and procedures required.
Knowledge of confidentiality protocols required.
Organizational and prioritization skills required.
Problem‑solving and composure in high‑pressure situations required.
Liaison ability with partner agencies required.
Independent work with minimal supervision required.
Knowledge of Housing First, Trauma‑Informed Care, Conflict Resolution, Motivational Interviewing, Low Barrier Operations, and Prevention and Diversion preferred.
HMIS data collection and reporting experience preferred.
Bilingual speakers preferred.
Physical demands: frequent standing, walking, reaching, lifting, stooping; moderate to loud noise; exposure to bodily fluids; contact with animals/pets possible.
