Overview The Case Manager is responsible for working closely with the Therapist(s) to coordinate services for residents within the facility and to assist with the resident’s transition from the facility. This includes coordinating academic, vocational, social/recreational, and independent living needs within the community for clients within HomeSafe’s residential group homes. Some evenings and weekends are required to fulfill the responsibilities of this position.
Responsibilities Support new client admissions by coordinating intake documents, assessments, and case planning within required timeframes.
Build and maintain positive relationships with community partners, case managers, guardians, schools, and other stakeholders.
Act as the education liaison for assigned clients, attending IEP meetings, monitoring academic progress, and coordinating with schools.
Develop, implement, and monitor independent living and life skills treatment goals through one-on-one sessions, group facilitation, and community resources.
Maintain accurate and timely client records, including treatment plans, progress notes, visitation logs, and court-related documentation.
Facilitate weekly and monthly meetings with clients and treatment teams to review progress, goals, and areas of improvement.
Coordinate and support family/sibling visits, court hearings, staffing meetings, and outside service provider sessions.
Assist clients in preparing for discharge by linking them with resources, supporting employment readiness, and ensuring identification and life skills needs are met.
Provide aftercare support and follow-up with clients post-discharge to encourage stability and continued progress.
Participate in staff training, program development, and coverage of daily program operations as needed.
Qualifications Bachelor’s Degree in Social Work, Psychology, Counseling, or related field required.
Three (3) years relevant experience required. Casework experience preferred.
Valid Florida driver’s license is required.
