Position Summary
The Case Manager is responsible for delivering and coordinating comprehensive client services within a short-term, live-in facility for homeless individuals. The facility serves approximately 200 adult males from a MICA (mentally ill, chemically dependent) population. The ideal candidate is proactive, demonstrates strong initiative, and has experience working with clients facing complex behavioral health and social challenges.

Conduct client intake within 24 hours of admission, clearly explaining resident rights, responsibilities, and grievance procedures.

Complete psychosocial, vocational, educational, and housing assessments with accurate and timely documentation.

Maintain thorough and up-to-date case records, documenting services, progress, and outcomes.

Develop and implement individualized service plans; conduct periodic reviews for a caseload of approximately 15–18 clients.

Provide holistic case management, including coordination of housing, mental health, and substance use services.

Prepare housing applications and assist clients throughout the housing and interview process.

Facilitate individual and group counseling sessions, psychoeducational workshops, and recreational activities.

Conduct regular room and dorm inspections to assess hygiene, cleanliness, and daily living skills.

Monitor client behavior, progress, and goal achievement, noting any changes or concerns.

Coordinate referrals for in-house and community-based medical, mental health, and social services.

Encourage client participation in meals and ensure appropriate nutritional habits are maintained.

Deliver effective crisis intervention and respond appropriately to emergencies.

Oversee medication compliance in collaboration with nursing staff.

Manage client discharges and transfers, ensuring proper documentation and continuity of care.

Escort clients to external appointments, housing interviews, or other essential meetings as needed.

Participate in staff meetings, case conferences, supervision, and required trainings.

Maintain current CPR/First Aid certification .

Adhere to all organizational policies, procedures, and applicable regulatory standards.

Demonstrate flexibility to work evenings or weekends as required.

Required Education & Experience

High School Diploma or GED required, bachelor’s degree in human services or related field preferred.

Proven ability to work effectively with a culturally diverse population.

Preferred Qualifications

1–2 years of experience working with homeless or high-need populations.

Experience in residential settings, group facilitation, tenant organization, case management, referrals, or community outreach strongly preferred.

Familiarity with DHA, CARES, mental health services, and substance use disorder treatment highly desirable.
