BHcare is seeking a Community Case Manager to provide compassionate, trauma-informed care to survivors at high risk, helping them reclaim safety, stability, and self-determination.
BHcare is a Certified Community Behavioral Health Clinic offering integrated care for individuals and families navigating mental health, substance use, and domestic violence. Our dedicated teams work to create lasting change through support, empowerment, and connection to vital services.
Note: This role requires regular travel within the community and the use of your own vehicle to support clients.
What You’ll Do
Provide direct, trauma-informed case management and safety planning to survivors of domestic violence.
Connect clients and their families to essential resources, including housing, legal aid, mental health care, and employment services.
Collaborate with law enforcement and partner agencies to enhance client safety and access to support.
Use tools like the Danger Assessment and VIGOR to develop individualized service plans.
Maintain accurate case records, participate in program reporting, and support continuous improvement efforts.

Qualifications
MINIMUM QUALIFICATIONS:
Bachelor’s Degree required
Bi-lingual (English/Spanish) highly preferred
Two years Advocacy and/or direct service experience working with survivors of domestic violence in a community-based setting preferred
Experience assisting traditionally underserved populations with a developed understanding of issues of diversity essential
Crisis intervention and counseling skills
Knowledge of community resources
Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
Must be able to obtain and maintain the domestic violence counselor certification per CGS 52-146K before providing direct service to clients, followed with six-hour recertification yearly
Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
