Case Manager Job description Overview
Alaska Addiction Rehabilitation Services Inc is a non-profit organization dedicated to providing compassionate, client-centered care for individuals on their journey to recovery from substances. Our team is passionate about making a difference in the lives of those we serve, and we are seeking a skilled and empathetic Case Manager to join our mission-driven team. The ideal candidate will have a strong background in behavioral health, substance use disorder with a focus on providing support to individuals facing various challenges and dual diagnosis issues. As a Case Manager, you will play a pivotal role in guiding clients through their healing journey.
Duties
Provide case management to clients, assisting clients in identifying case management services; as needed, in accordance to best practices.
Accomplish clients care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches.
Maintain client records in a timely, orderly manner according to the guidelines established in the agency treatment manual, reviewing case notes; logging events and progress, and documenting all billable interactions into AKAIMS, utilizing DAP note formatting.
Perform intake assessment interviews using the bio-psychosocial assessment documents, the DSM-IV Diagnostic Criteria and the ASAM: reviewing counselor evaluations, treatment objectives, and plans.
Qualifications
Have High School Diploma or GED, an associate degree in human services or related field is preferred.
Have a QAP equivalent certification.
Experience working in a residential setting preferred.
Must be knowledgeable of the interaction between chemical dependency and mental illness and the treatment issues related to dual disorders.
Must be able to communicate effectively verbally and in written form.
