La Casa Norte’s mission is to serve youth and families confronting homelessness. We provide access to stable housing and deliver comprehensive services that act as a catalyst to transform lives and communities. Our programs, including Casa Corazón (Drop-In and Emergency Beds) and Solid Ground Supportive Housing, aim to transform the lives of homeless and at-risk youth (ages 16–24) by providing them with the stability, support, and guidance they need to thrive.
Job Summary The Case Manager is responsible for providing comprehensive case management, advocacy, and supportive services to homeless and at-risk youth. This position supports youth in developing life skills, securing stable housing, and accessing educational, vocational, and healthcare resources. Work schedules vary depending on program assignment, typically Monday–Friday with evening and weekend flexibility required. Positions are located across multiple neighborhoods, including Humboldt Park, Back of the Yards, and Logan Square.
Key Responsibilities Advocacy and Support Services
Provide case management using a strength-based, trauma-informed, harm reduction approach.
Maintain professional, supportive relationships with youth, staff, and community partners.
Assist youth in overcoming barriers to housing, employment, and education.
Deliver crisis intervention services and ongoing mentoring.
Advocate for youth with landlords, schools, employers, shelters, and institutions.
Refer and connect youth to wrap-around services (e.g., food pantries, clothing closets, legal aid, parenting classes, mental health/substance use services).
Case Management Duties
Conduct intakes, assessments, and individualized service/action planning.
Assist with applications for public benefits and other documentation.
Support youth in identifying and securing housing opportunities (shelters, supportive housing, apartments).
Collaborate with internal and external partners to provide holistic care.
Document all services and outcomes in HMIS, ClientTrack, and paper files.
Program Operations and Environment
Support coverage during program hours and coordinate workshops, food, and activities.
Maintain safe and welcoming environments, addressing safety hazards, emergencies, or incidents.
Encourage youth participation in maintaining clean, healthy shared spaces.
Plan and facilitate monthly schedules of programming and activities.
Support volunteer groups and community engagement events.
Administrative and Leadership Responsibilities
Assist Program Supervisors/Coordinators with program development and service delivery.
Provide supervision to interns, volunteers, and graduate-level students as appropriate.
Participate in on-call rotation, covering occasional evenings and weekends.
Assist with data collection, grant reporting, and audits.
Uphold La Casa Norte policies, including the Declaration of Non-Violence.
Professional Development and Team Collaboration
Attend weekly staff meetings, trainings, and workshops.
Participate in La Casa Norte’s fundraising and outreach events.
Collaborate with colleagues to strengthen and develop new programming.
In addition to the responsibilities listed above, all La Casa Norte employees are responsible for contributing to the overall success of La Casa Norte by:
Participating in La Casa Norte’s fundraising events.
Participating in on- and off-site staff trainings, workshops and outreach events, as needed.
Collaborating in a team approach to develop new programs.
Participate in on-call rotation to support all youth programs overnight and on weekends.
Perform other duties as needed.
Qualifications and Experience Bachelor’s degree in social work, psychology, or a related field or at least three (3) years of equivalent experience.
Bilingual English/Spanish strongly preferred.
Demonstrated ability to work with homeless, at-risk, LGBTQ+, and adjudicated youth.
Knowledge of Latino and African American cultural, social, and economic contexts.
Strong organizational, time management, and communication skills.
Ability to work independently and collaboratively in fast-paced environments.
Valid driver’s license, insurance, and reliable vehicle required.
Must pass background check and medical clearance if assigned to certain programs.
Work Conditions Flexible hours, including evenings, weekends, and on-call rotation.
Must be able to walk up and down stairs and lift/move up to 25 lbs.
Frequent engagement in community outreach, program activities, and events.
