This position is a dual role, serving as a Case Manager for a Christian non-profit organization, Shepherd's Heart Fellowship, while also acting as a Veterans' Services Leader. The primary goal is to assist homeless veterans in achieving stable living arrangements. This involves providing individualized support, connecting clients with resources, and advocating for their needs. The role requires a personal faith in Jesus Christ as Lord and a heart to minister to others. This successful candidate will report directly to the Pastoral Counselor.

Key Responsibilities

  • Client Support & Case Management:
    • Assess clients' physical and mental wellness, needs, preferences, and abilities to develop tailored care plans that align with VA requirements.
    • Meet at least weekly with each resident to assess their progress and address any issues that may be delaying their SMART goals.
    • Assist veterans in qualifying for various assistance programs and assess potential landlords for housing placements.
    • Follow up with discharged clients to ensure they are satisfied with services and are in good physical and mental health.
    • Transport veterans as needed to various appointments and activities.
  • Resource and Networking:
    • Establish and maintain relationships with the VA's Healthcare for Homeless Veterans program, HUD-VASH, and other veterans' programs, as well as county housing authorities.
    • Ensure veterans are referred for ongoing services they may need for independent living, such as medical and mental health services, vocational rehabilitation, recovery services, and financial management.
    • Collaborate with external agencies who provide essential services for the veterans, their families, and the Shepherd’s Heart Veterans’ Home Ministry.
    • Prepare and submit all required reports and data to the VA and Finance Department in a timely manner, including quarterly reports and data for the Homeless Management Information System (HMIS).
  • Documentation and Record-Keeping:
    • Maintain accurate and detailed case records, including charting referrals and activities directly related to the veteran’s SMART Goals and Individual Service Plan.
    • Supervise and monitor all documentation activities to ensure client records meet VA and Quality Improvement standards.
    • Develop, monitor, and amend various forms such as intake, discharge, budget, individual service plans (quarterly), and monthly process notes.

Qualifications

  • Education: A Bachelor of Social Work (BSW) or a related field such as social work, counseling, psychology, or ministry is required. An associate degree in a social services-related field with at least 2 years of experience will be considered. Extensive experience in case management, social services, or addictions recovery, may substitute for formal education.
  • Experience: A minimum of 2 years of administrative or supervisory experience is required. The ideal candidate will have experience working with homeless adults, particularly veterans, and be knowledgeable about human service resources in Western Pennsylvania and VA.
  • Skills & Qualities:
    • Demonstrate a compassionate, flexible, and organized approach.
    • Possess strong verbal and written communication skills to explain care plans and maintain good records.
    • Exhibit sound judgment, critical thinking, and problem-solving skills to determine the best care plan for each client.
    • Have the ability to respond appropriately in a crisis situation.
    • Valid Driver’s License.
    • Successful completion of background check and clearances.

Work Schedule

  • Work Week: Full-time hours are expected to be 40 hours per week. Although unlikely, this may include working limited evenings and weekends for special events and staff support.
