Overview
The Case Manager is part of a human service team that develops and executes service and care plans for residents to provide a supportive and empowering environment that fosters stabilization, independence and self-reliance, and skills development. The Case Manager plays a key role in executing and updating service and care plans, coordinating direct services and service referrals to external providers, and monitoring resident behaviors and practices.

Pay and Schedule
Pay: $21.64 per hour (approx. $45,000 annually). FLSA: Non-exempt. Eligible for overtime.

Schedule: Monday-Wednesday, Friday 9am-5pm; Thursday 11am-7pm. Must work 4th Saturday of the month 9am-5pm. Some positions may have alternative schedules (e.g., Monday - Friday 3pm-11pm with 1st Saturday of the Month).

Location: Gowanus / Park Slope, Brooklyn, NY.

Responsibilities

Service and care coordination: ensure residents receive appropriate quality services per organizational principles and program requirements.

In coordination with Social Worker (if available), design service and care plans that provide counseling, crisis intervention, direct service, advocacy, and linkages to external and community-based services.

Collaborate with other service providers and external organizations to help residents resolve immigration and citizenship issues, secure public benefits, pursue employment and housing stability.

For family sites: coordinate with Recreation Team to ensure transportation, school transfers, ESL/ELL and IEP as needed.

Develop and maintain a resource network for referrals related to resident service needs.

Meet with residents weekly to assess progress and modify plans as needed.

Conduct weekly room inspections to maintain health and safety; coordinate with relevant teams to mitigate concerning behaviors.

Design discharge plans to ensure continued enrollment and linkages to public benefits and community-based services.

Benefits / Entitlement Coordination: enroll residents in eligible public benefits and city programs per DHS requirements; ensure follow-through with appointments and documentation.

Coordinate with immigration-related organizations and enroll residents in immigration services if applicable.

General duties: document interactions and communications; perform other duties as assigned by supervisor and Program Director.

Qualifications
Education and Experience: High School Diploma or GED required. Bachelor’s degree in social work or psychology preferred. Three years of relevant experience can substitute for education.

Two years' experience in case management, counseling, or direct advocacy for clients in social service settings. Experience with immigrants and immigration issues is a plus.

Excellent verbal and written communication skills. Proficiency in English required; multilingual and Spanish proficiency a plus.

Basic computer skills (MS Word, Excel, Outlook, Internet).

Ability to formulate simple financial budgets; ability to prioritize and make sound decisions.

Physical ability to perform unit visits and navigate buildings (walking, stairs).

Experience working with homeless individuals and people with health/mental health or substance use issues; ability to engage residents.
