Case Manager
Location: 97 Crooke Ave, Brooklyn, NY 11226

Responsibilities
Review and maintain tenant files and documentation.
Conduct initial intake, periodic assessments and independent living plans.
Assist tenants in accessing community resources and referrals.
Monitor progress, provide progress notes and complete required reports.
Participate in case conferences and liaison with external organizations.
Provide additional case management duties as outlined (e.g., escort appointments, benefit application assistance).
Qualifications
Bachelor’s degree and experience working with the Seriously and Persistently Mentally Ill population.
Flexible hours may be needed occasionally to provide shift coverage until 8 pm.
Compensation
$40,000-$45,000 annually
