Case Management RN Manager

The Case Management RN Manager is a Registered Nurse responsible for overseeing the day-to-day operations of the RNs within the Care Coordination Department. Responsible for timely coordination of quality services to address a patient's specific needs in a cost-effective manner to promote positive outcomes. Responsible for assessing, planning, implementing, monitoring, and evaluating patient factors with a focus on discharge planning.

Minimum Qualifications:

  • Graduate of accredited school of Nursing, Baccalaureate degree preferred.
  • Current RN licensure by the Louisiana State Board of Nursing with three-five years of hospital adult acute care nursing experience.
  • Current BLS certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: American Heart Association and American Red Cross. The ILCOR course must include a manikin check off.
  • CCM, CMAC, or ACM preferred or must be obtained within 18 months of hire.
  • Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.
  • Basic understanding of Medicare, Medicaid, and Managed Care regulations.
  • Organized and efficient in establishing priorities and performing tasks within specific timeframes.
  • Self-motivated requiring minimal supervision.
  • Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress.
  • Responsible for maintaining strict confidentiality of all verbal and written information and documents.
  • Familiarity with and appropriate use of community resources.

Physical Demands:

Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 20 pounds is required. Must be able to work with a moderate level of noise.
