Position Title: Case Manager
Location: Calabasas, CA
Setting: Detox / Residential Treatment Center
Population: Adolescent, Dual-Diagnosis, Co-Ed
Schedule: Full-Time | 8:00 AM – 4:00 PM


Position Overview

The Case Manager plays a critical role in supporting the treatment and recovery process for adolescents struggling with mental health and substance use disorders. This position is responsible for coordinating individualized care, maintaining communication between clients, families, and treatment teams, and ensuring each client’s treatment plan is implemented effectively throughout their stay.


Key Responsibilities

  • Conduct comprehensive biopsychosocial assessments and assist in the development of individualized treatment plans.
  • Collaborate closely with clinical, medical, and support staff to ensure continuity of care.
  • Serve as the primary point of contact for clients and families, providing updates, education, and emotional support throughout treatment.
  • Coordinate discharge planning, including aftercare referrals, outpatient follow-ups, and community resources.
  • Facilitate case conferences and participate in daily team meetings to review client progress and modify treatment goals as needed.
  • Maintain timely, accurate, and thorough documentation in the facility’s electronic health record (EHR).
  • Monitor client compliance with program guidelines and support the treatment team in addressing behavioral or clinical concerns.
  • Communicate regularly with outside providers, insurance representatives, and referral sources as appropriate.
  • Advocate for clients’ needs while maintaining professional boundaries and confidentiality in accordance with HIPAA and facility policies.


Qualifications

  • Bachelor’s degree in Social Work, Psychology, Counseling, or related field required (Master’s preferred).
  • Minimum of 2 years’ experience in behavioral health or substance use treatment (experience with adolescents strongly preferred).
  • Working knowledge of DSM-5 diagnostic criteria and evidence-based treatment modalities.
  • Strong organizational, communication, and crisis-intervention skills.
  • Familiarity with documentation systems such as KIPU or similar EMR platforms.
  • Current CPR/First Aid certification (or willingness to obtain).


Compensation

Pay Rate: $24 – $26 per hour (depending on experience)
