Position Summary
Case Managers work in collaboration with the clinical team to identify, coordinate, and secure community‑based resources to support the adolescent client’s ongoing treatment and recovery. The case manager also facilitates the admission and discharge process, coordinates outpatient care, and maintains up‑to‑date knowledge of relevant community resources and the procedures for accessing services from external providers and social agencies.

Responsibilities

Facilitates the admission process by partnering with the admissions team and site staff to arrange for client needs.

Assesses post‑discharge needs and communicates client discharge needs to the treatment team; may partner with families or other supports to coordinate discharge plans.

Partners with Accanto staff to coordinate internal steps to lower levels of care and with external step‑down providers and referring providers to coordinate client care including treatment appointments, lodging, and other community resources.

Documents in the electronic health record in accordance with Accanto policies and other applicable regulatory standards.

Maintains working knowledge of community resources and referral procedures; networks with other organizations to foster referral relationships.

Qualifications

Bachelor’s degree in mental health related field required; Licensed Social Worker preferred.

Mental health case management experience preferred.

Must have a valid driver’s license and be authorized to drive.
