Position Summary
The Case Manager supports the Enhanced Care Management Programs by providing care coordination/case management services to enrolled clients. The ultimate goal of all services provided is assisting the client with obtaining a medical care health home, an income, medical insurance, social services, and housing. These services include assistance with co-occurring disorders (mental health and substance abuse) with the expectation of improving current functioning level and skills acquisition through assessment, engagement, assisting with access to treatment including medical care with a primary care provider, rehabilitation and goals. The Case Manager will complete intakes of all clients, document progress notes and maintain client records in the patients/clients Health Action Plan. Additionally, the Case Manager will transport clients and coordinate linkage to various community resource programs.
Required Qualifications
High School diploma or equivalent, 1 year case management experience and/or lived experience utilizing Medicaid or other social service systems required. Bachelor's degree preferred.
Must be able to work collaboratively with others in a team environment, respecting and valuing the perspectives and experiences of others.
Familiar with Harm Reduction, Motivational Interviewing, Client Centered theory and Trauma Informed Care.
Able to maintain and understand appropriate roles, and boundaries among clients, peers, and WellSpace partners/stakeholders.
Able to function effectively in a fast paced environment.
Ability to write progress notes and maintain detailed, statistical records.
Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
Must demonstrate a high level of verbal, writing and listening skills.
Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
Current CPR and first aid certificate or willingness to be trained required within first (90) ninety days of employment.
Computer literacy including use of email, Microsoft Word, Excel, and El-IR.
Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
Assist and support the Center/Department/Program to meet standards of High Reliability.
Must be willing to transport clients on a daily basis.
Must have valid driver's license and insurable driving record.
Meet with each client individually and conduct an intake needs assessment.
Represent and/or advocate for and with clients in obtaining identification, housing, benefits, services, and entitlements from governmental and private agencies and organizations.
Coordinate linkage to various community resource programs and supports.
Collaborate with the client/patient and care team in the development of an individual health action plan and support client/patient in the execution of the plan.
Write and enter progress notes daily into an electronic patient care system.
Attend regular meetings with management and team.
Perform other duties as assigned.

Physical Demands and Work Environment
The work environment is characteristic of a typical office environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands and arms to reach. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.
The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
