Position
We are looking for a Case Manager, who is willing to work in conjunction with the Caseworker Manager to identify strengths, needs, and resources to support UCs in Post Release Services. The Case Manager will assist in coordinating the Individual Service Plans for UC children to aid in the stabilization of UCs in the homes of their sponsors.

The pay range we’re offering is $68,640 annually, depending on experience.

Qualifications
PRS Case Managers must have knowledge of local community social services
PRS case managers must have a BSW or an equivalent degree in education, psychology, sociology, or other behavioral science
PRS case managers must be bilingual in English and Spanish or have access to professional interpretation services
Specialized experience working with immigrant populations preferred
Experience working with family preservation, kinship care, and/or general child welfare preferred
Proficient in Microsoft Office Suite (MS Word, Excel, PowerPoint, Outlook)
Possess superior oral and written communication skills and interpersonal skills
Detail oriented and organized
Able to work independently and organize multiple projects with minimum supervision
Able to work as a team member
Able to maintain the highest level of confidentiality and professional boundaries with clients and staff
Able to represent the agency professionally and appropriately with all constituencies
Must pass pre‑employment health screen including physical exam, TB and drug screen
Ability to drive to various sites on and off campus
Ability to lift 10 pounds for filing
DOJ, FBI, Child Abuse Index; Insurability under corporate automobile insurance
Key Responsibilities
Case Manager will maintain a caseload of Level 1 and Level 2 UCs in the community
Support UCs and their Sponsors in identifying strengths, needs, and community resources to promote stability in the community
Responsible for completing appropriate documentation, assist with the implementation of the UCs Individual Service Plans
Completing all case management related documentation in the UC Portal within the required timelines
The ability to implement case plans through good professional judgment while maintaining a team approach is essential
Crisis intervention and client de‑escalation skills are necessary and valued
Liaison with the UCs, FFS, families and sponsors
Other related duties as assigned
