SUMMARY: The Case Manager provides direct case management services while engaging homeless individuals with appropriate concern for their social, mental health, substance abuse and medical problems in order to promote client stability and empower them to reach their fullest potential.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Completes intakes for new clients and provides an orientation to the program and shelter rules and regulations
Reviews logbooks at the start of each shift to ensure that all clients follow-ups are conducted as required
Participates as part of a team and regularly collaborates with multidisciplinary staff to monitor clients’ progress
Develops a comprehensive psychosocial assessment to fully identify and address the client's needs
Collaborates with internal staff to monitor clients’ medication compliance and address episodes of non-adherence
Establishes an individualized intervention/treatment plan or strategy to promote identified goals
Provides advocacy for individual clients around entitlements and services
Provides case management, including individual counseling, crisis intervention, conflict resolution, advocacy, psychoeducation, coordination of services and appropriate follow-up on all referrals
Uses motivational interviewing and seeks safety modalities to counsel clients to improve overall effectiveness
Encourages and provides ongoing support so that each client might assume an active role in his or her own treatment plan to the maximum extent possible
JOB QUALIFICATIONS:
Educational Requirements:
Bachelor degree in a Social Work, Psychology, Counseling or another related field preferred
Experience:
Three (3) to five (5) years of case management experience with the homeless, mentally ill, substance abuse and/or low-income populations preferred
Skills Set:
Strong relationship-building skills required
Ability to work independently but know when to raise issues to the supervisor
Excellent organizational and time management skills necessary
Ability to effectively multi-task and follow up on a timely basis
Strong PC skills required including Word and Excel
Languages:
Bilingual in English and Spanish strongly preferred
