Salary: $48,160

Hours: 40 hours per week

Schedule: Monday‑Friday 8:30am‑4:30pm

Summary: To provide direct care support and opportunities for individuals through supervision, role modeling and teaching. To help develop and maintain work and life skills and assist individuals to establish a network of relationships in the community that enrich their lives. Complies with all principles of Human Rights, D.P.P.C., Department of Developmental Services, the Division of Medical Assistance and all other applicable funding regulations as adhered to by Lifeworks.

Essential Duties and Responsibilities

Promotes optimal health for individuals through routine, preventive, and emergency health care. Ensures that work environments and Life Enhancement activities are safe, secure and free from harm.

Provides supervision, training and coaching across all settings to increase individuals’ self‑sufficiency and independence, meeting employers, volunteer sites, families and supported individuals. Supports employers as needed and ensures quality and consistency of work performed.

Provides education, training, counseling, guidance and physical assistance to achieve personal goals in employment, education, social growth, health and wellness, communication skills, community mobility and volunteerism.

Develops, writes and presents annual Individual Support Plans (ISPs) and related paperwork in consultation with supervisor. Advocates for ISP implementation to achieve goals through employment and the Life Enhancement Program.

Performs case management duties including documentation, maintenance of confidential files, submission of required paperwork, and attendance at regular meetings.

Remains flexible regarding changes in work schedule and tasks to cover individual needs.

Maintains regular communication with individuals, ISP team members and significant persons (family, guardians, advocates) to facilitate consistent programming. Acts as liaison between Lifeworks, employers, volunteer sites and community organizations.

Provides transportation to and from community sites using Lifeworks or personal vehicles. Reimbursed at agency mileage rate for personal vehicle use.

Demonstrates proficient computer skills and ability to learn agency programs.

Assumes other duties as required by supervisor.

Supervisory Responsibilities
This position has no supervisory responsibilities of other staff.

Qualifications

Education: BA/BS in Human Services related field or associate degree plus 3 years related experience with persons with ID/DD or Human Services; or HS Diploma/GED plus 5 years related experience.

Language: Ability to read and interpret documents, write reports, present effectively.

Mathematical: Ability to perform basic arithmetic, compute rates, create bar graphs.

Reasoning: Apply common sense to follow written or oral instructions and solve problems in standardized situations.

Vehicle/Travel: Legally insured and inspected automobile available during work hours. Agency vehicle use allowed.

Certificates/Licenses: Valid driver’s license; Medication Administration, CPR, First Aid.

Physical Demands
Regular standing, walking, sitting, using hands, reaching, talking/hearing. Occasionally climbing, stooping, kneeling, crouching, crawling. Must lift up to 50 pounds. Vision: peripheral, depth perception, focus adjustment.

Work Environment
Exposure to outside weather, wet/humid conditions, moving parts, vibration. Noise level moderate. Employees may have meal breaks with individuals served and may need to provide assistance.
