Responsibilities
Summary: Under close supervision, the Case Manager provides support and structure to youth in developing program plans by managing a caseload and administering the policies and procedures of the program as directed by the contracting agency and the Program Manager. Learns, uses and models the principles of the “What Works” system through evidence-based practices in all interactions with participants and other staff members.

Prepare written personalized programs for each youth to establish measurable criteria of expected behavior and a time frame for achieving specified goals.

Review plans with youths and document progress or revisions to ensure life skills development for successful re-entry into the community.

Assist youths to establish personal budgets, locate housing, enroll in educational and vocational programs, and participate in family and community activities to pursue financial independence.

Orient youths at facility arrival to ensure smooth assimilation into the program and complete required paperwork.

Enter confidential youth files to support case analysis and program planning while protecting youth confidentiality.

Verify youths' pass plan through pass sponsorship according to contract, ensuring accountability for whereabouts in the community.

Prepare discharge reports within established time parameters to summarize program performance and satisfy contract requirements.

Participate in staffing meetings to share observations and recommendations and contribute to team problem solving.

Maintain confidentiality of information obtained in the course of duties and report program violations per established parameters.

Assist with conducting orientation meetings for new participants and explain program rules and the participant contract.

Set participant schedules and approve activity-based probation adjustments; communicate effectively with all levels of staff to promote teamwork.

Assist with safety compliance and adhere to attendance, safety, and policy requirements; perform other duties as assigned.

Qualifications
Minimum Requirements

High School Diploma or equivalent; a Bachelor’s degree in Social Science or Human Services, or equivalent experience, is preferred.

One (1) year of experience as a Case Manager or two (2) years of related work experience.

Familiarity with community corrections systems and practices.

Ability to write clear, concise reports in English with correct grammar.

Ability to perform basic mathematics (add, subtract, multiply, divide, calculate percentages).

Ability to react quickly and appropriately in emergencies.

Bilingual in English/Spanish preferred, but not mandatory.

Proficiency with computers and department software.

Ability to work in an in-custody setting.
