Location: Providence Hwy, Norwood, MA

Annual Salary: $48,160.00

Hours: 40 per week

Schedule: 8‑hour shift; Monday‑Friday: 8:00 am‑4:00 pm; Friday: 7:30 am‑3:30 pm

Job Overview
As a Case Manager at Lifeworks Employment Services (LES), you’ll facilitate community and program-based activities to encourage self‑advocacy and skill building. Your leadership will enhance individuals’ volunteer and employment experience. You will provide job coaching, guidance, and physical assistance when necessary to achieve a person’s individual and employment goals. Through direct support, you will motivate people we serve to be empowered in their programs and communities and will have the ability to make a real difference in the lives of people with intellectual and developmental disabilities.

Responsibilities

Collaborate with an exceptional team to plan and engage in advocacy and support of individuals, attending regular meetings as necessary.

Support individuals’ self‑employment, education, social growth, health and wellness, communication skills, community mobility, and volunteerism.

Provide support through case management (electronic and physical case records).

Write and implement Individual Support Plans (ISP) and complete all documentation.

Provide monthly and weekly progress notes for individual goals and objectives, maintaining consistent communication with individuals and the ISP team.

Safely operate company van within legal Massachusetts guidelines; assist individuals in meeting employer expectations at GSWS.

Provide on‑ and off‑site support for Life Enhancement Curriculum (assisting individuals in employment, social, creative, and health & wellness development and training).

Lead individuals in community volunteer and learning experiences; complete daily payroll and billing documents.

Perform other duties and assignments at various locations as needed.

Qualifications

Bachelor’s or Associate’s degree in a related field with at least three years of experience, or five years related work experience in Human Services.

Effective oral and written communication skills.

Valid driver’s license and ability to pass a CORI, reference, multi‑state background, and driving record check.
