Overview Join to apply for the Case Manager role at Acadia Healthcare .
This is a full-time, benefit-eligible position working Monday - Friday, 8:30 am - 5:00 pm. The role involves managing clients' cases with clinical knowledge and may include providing therapeutic services and facilitating groups for clients.
Responsibilities Provide case management and care coordination services for clients in the facility.
Coordinate continuum of care activities for assigned clients, ensuring optimal resource utilization, service delivery, and compliance with external agencies and referral sources.
Conduct psychosocial assessments and individual, family and group sessions.
Coordinate treatment and discharge activities as needed.
Develop, implement, manage and evaluate client/patient care plans to ensure quality care.
Collect, correlate and provide clinical data to the treatment team.
Convey medical criteria and clinical information between the insurance provider and the treatment team as warranted.
May correlate clinical data for the business office as required.
Coordinate issues or activities relevant to the treatment team in a timely manner.
Qualifications Bachelor’s degree in Social Work, Counseling or related human service field required.
One or more years’ experience working in a behavioral health setting preferred.
Previous experience in behavioral health preferred.
Previous experience in case management and/or utilization review preferred.
