Overview Case Manager role at High Point & Affiliated Organizations in New Bedford, MA. Part-time position (Monday - Friday, 3:00 PM to 7:00 PM or 4:00 PM to 8:00 PM). Pay range: $22 - $27 per hour (commensurate with experience and education).
Responsibilities & Duties Maintain safety and security of residents and the facility.
Ensure timely contact with Program Manager and authorities in the event of a crisis or emergency.
Understand and follow established referral procedures and schedule intakes; facilitate intakes of new residents including setting up new records and completing intake paperwork.
Complete Initial Individual Service Plan (ISP) with resident; coordinate referrals to appropriate agencies/providers (including but not limited to: DTA, SSA, DCF, Medical/Mental Health agencies, etc.).
Maintain accurate case files; obtain required authorizations to release/obtain information; prepare legible case management notes; document referrals on designated forms; update ISPs monthly; ensure periodic evaluations are completed timely.
Record resident attendance at meetings, appointments, groups, etc.; meet with each resident bi-weekly to review progress and determine if needs are being met; revise/update ISP as needed and establish additional goals/objectives and steps to achieve them.
Attend monthly case reviews with the Program Manager and participate in clinical supervision, staff meetings, and trainings; ensure timely completion of Admission/Discharge satisfaction surveys by residents.
Coordinate and arrange transportation as needed; perform other duties as assigned by the Program Manager.
Qualifications High School Diploma or GED; one year of experience in the substance use treatment field preferred.
Valid Driver's License.
Knowledge of the relationship between substance use and mental health issues and/or self-help programs.
Understanding and ability to maintain therapeutic boundaries; reinforce rules in a respectful manner.
Ability to work with others in a cooperative and collaborative manner.
Knowledge of basic computer skills and software programs.
Must comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and HIPAA Regulations.
Certifications: TB/Flu and COVID-19 assessment; de-escalation program; CPR; and successful completion of all trainings upon hire and annually as required.
