Case Manager Job Responsibilities
Oversee, maintain, monitor, and supervise a caseload consisting of 30-40 consumers.
Coordinate necessary meetings and ensure appropriate documentation at meetings (30-day Review, 6th Month Review, Pre-ISP, ISP, Case Conference, etc.).
Monitor and track adaptive equipment, reporting any damages, breakages, or losses to DDS Adaptive Equipment Coordinator, DDS Service Coordinator, and Residential Provider.
Ensure that mealtime protocols are being followed and notify appropriate persons when necessary.
Create person-centered long-term objectives and goals.
Ensure that contact information for each person is updated and accurate.
Complete annual documents needed for ISP meetings (Functional Analysis Assessment, PPP, Personal Centered Thinking Tool, LON).
Monitor and respond to MCIS Dashboard issues.
Create, distribute, and upload quarterly reports according to DDS policy.
Send reports to Court Representatives by the assigned due date.
Ensure outside agencies receive necessary documents and reports.
Notify legal guardians and/or family members about Human Rights Committee Meetings.
Complete Therap Quarterly Reports in a timely manner.
Use Therap progress notes to create monthly summaries for quarterly reports.
Check ISP amendments daily.
Act as a liaison/advocate for PRD and all waiver services provided by the company.
Print and monitor Service Authorizations and communicate with the Accounting Department.
Communicate with DDS personnel regarding SA and PA issues.
Print ISP and all supporting documents in a timely manner.
Request, receive, and distribute correspondence from outside providers to appropriate departments.
Ensure completion of annual ISP Training, Emergency Staffing Training, and new employee ISP Trainings (Phase II Training).
Train program staff on person-centered goals and objectives.
Create community outing letters for residential providers.
Sign and approve community outings.
Ensure program staff complete daily body observations, monthly attendance, and interval documentation.
Communicate major status changes in goals to DDS Service Coordinator for ISP amendments.
Support staff during spot treatment studios and fire drills.
Assist with other work tasks as needed.
Attend DDS trainings, presentations, and forums.
Serve as a mandatory incident reporter for witnessed, informed, or discovered incidents.
Attend management meetings.
Follow all PRD policies and procedures, complete PPD tests, and maintain CPR, First Aid, AED certifications.
Contribute to and maintain a positive work environment and attitude.
Utilize talents and skills to support team efforts.
Perform other duties as assigned.
