Role Description This is a full-time, on-site M-F, 9 am-5 pm role located in Providence, RI, for a Case Manager. The Case Manager will be responsible for coordinating and managing care plans for elders and long-term support services, acting as a liaison between clients and service providers, and ensuring clients receive appropriate resources and support. Additional responsibilities include maintaining accurate client records, conducting assessments, monitoring client progress, and providing guidance to optimize overall wellness.

Proficiency in case management, needs assessment, and resource coordination.
Organizational skills and the ability to manage multiple clients and tasks simultaneously.
A collaborative mindset with the ability to work independently and on a team.
Previous experience in case management or a related field.
Familiarity with local resources is a plus.
Commitment to client confidentiality and ethical practices in accordance with federal and state regulations.
Must be able to travel throughout Rhode Island; mileage reimbursed.
