Position: Community Services Case manager

Reports to: North Region Associate Director

Supervises: None

Department: Community Services

FSLA Status: Non-Exempt; Part time

Position Summary:

Provide case management services and related support to adults and families in various programs within the agency that may include but are not limited to:

  • Resource & Referral including Thrive life skills classes and Christmas Program
  • Education Creates Hope and Opportunity (ECHO)
  • Pregnancy & Parenting case management
  • Indiana Trafficking Victims Assistance Program (ITVAP): case management support for victims of trafficking
  • Retired Senior Volunteer Program (RSVP): food pantry, VITA

The case manager must be able to communicate well with their clients and have a solid understanding of how to work with people from diverse backgrounds.

Schedule:

We prefer someone who can open (8am) or close (5pm) Monday-Thursday. Initially looking for part-time candidate could be full-time in the future with added caseload.

Essential Duties/ Responsibilities:

To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below.

  • Provide direct case management services to clients in office, home, community setting including assessments, case planning, advocacy, referral, crisis management, skills development and problem solving as needed
  • Assists pregnant and parenting teens with addressing barriers faced in completing their high school education or high school equivalency program and/or post-secondary program completion
  • Represent the ECHO program with community stakeholders or service providers.
  • Provides Resource/Referral direct services to clients in need of emergency financial or material assistance or enrolled in self-sufficiency programming
  • Assist clients as may be needed to access public assistance
  • Assist with coordination of activities in support of food pantry operations including food or donation pick-up, food drives, volunteer interaction, inventory maintenance or other projects
  • Provide one on one or small group skills building or training for clients including budget review, financial literacy, banking, job skills development, job search and interviewing practice, school enrollment, FAFSA completion, etc.
  • Provide direct service and coordinate activities for clients impacted by disasters
  • Making program purchases as directed
  • Develops and maintain relationships with school and community service providers in order to coordinate client services
  • Promote and represent the programs in the community and with clients
  • Attends relevant community service trainings, committee and service provider meetings
  • Ensures that all case documentation and case records are current and accurate
  • Assists in the collection of data, preparation of grants and program reports as may be needed
  • Comply with all grants, funder and audit requirements of the program
  • Ensures that the services provided are consistent with the agency’s mission and philosophy.
  • Respect for Catholic Social Teaching and compliance with the agency code of ethics.
  • Perform related functions necessary to support the mission and core values of Catholic Charities.

Minimum Job Requirements:

  • Bachelor’s degree in social service or related field from an accredited college/university.
  • One year work experience in social service setting preferred
  • Working knowledge of community service and public program providers
  • Working knowledge of CORE 40/ HSE requirements, college application & entrance requirements and FAFSA application process
  • Working knowledge of employment skills development and financial literacy
  • Ability to work independently and effectively to provide services with limited supervision
  • Ability to effectively represent the program in public situations
  • Excellent written and oral communication skills
  • Ability to travel in personal vehicle within the North Region counties on a regular basis and within the Diocese of Fort Wayne South Bend, Inc. as needed

Conditions of Employment:

  • Valid Driver’s License with clean driving record.
  • Reliable vehicle with personal vehicle liability insurance coverage
  • Proficient in Internet, word processing, spreadsheet, and e-mail applications (prefer Microsoft Office).
