Case Manager

Job Title: Case Manager – Community Support Services

Location: Port Angeles, WA (On-site only)

Job Type: Full-time

Schedule: Monday to Friday, 40 hours/week

Pay: $24 to $27 per hour (DOE/DOQ)

As a Case Manager in our Community Support Services department, you’ll provide direct support to clients through individualized service planning, advocacy, and coordination of care. This is a dynamic role for someone passionate about behavioral health and community impact.

What You’ll Do

• Deliver person-centered case management services.
• Support clients in accessing housing, food, transportation, and employment.
• Collaborate with hospitals and community agencies.
• Provide psychoeducational and supportive counseling.
• Maintain compliance with documentation and confidentiality standards.

What We’re Looking For

• Bachelor’s degree in Behavioral Science or related field
• 2+ years of experience in case management or clinical services
• Valid driver’s license and insurable driving record
• Ability to pass pre-employment screenings and provide vaccination records (MMR, Hep B, Tdap, flu)
• Agency Affiliated Counselor Registration credential (assistance provided)
