Sentara Health is seeking to hire a qualified individual to join our team as a Care Transition Coordinator.
Position Status: Full Time - Day Shift
Position Location: Woodbridge, VA
Hourly Range: $32.39 - $53 We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits.
Care Transition Coordinator (CTC) serves as the liaison between inpatient providers, discharge planners, and our home-based services. This high-visibility role combines clinical knowledge with strategic sales acumen, playing a critical role in identifying appropriate patients, educating care teams, and influencing discharge decisions to support optimal post-acute transitions. CTC builds and maintains strong relationships with referral sources and educates patients/families on available services. Collaborates with operational teams to ensure timely admissions and optimal patient experience.
The ideal candidate is a results-oriented professional who understands the nuances of referral development, is comfortable navigating acute and post-acute settings, and thrives in a relationship-driven environment.
Education: High School Diploma (Required)
Certification/Licensure: RN, LPN, BSW/MSW, LPTA, RT (Required)
