Region IV Area Agency on Aging is seeking a licensed RN or Social Worker Care Manager for an open Care Management Position in our MI Choice Waiver program! MI Choice is a program that allows eligible long-term care patients to receive services in their homes. The primary goal of the Area Agency on Aging is to support older adults in aging in place as long as they want and are able, by helping them connect with various services throughout our service area.
Our service area covers Berrien, Cass, and Van Buren counties in southwest Michigan
SUMMARY: Works with adults ages 18 and older, their families, and caregivers. Performs comprehensive biopsychosocial assessments and develops, implements, and monitors person-centered plans of care coordinating home and community-based long-term supports and services.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provides ongoing information and support to caseload of clients and their families and caregivers.
- Maintains strong relationships with partners and providers throughout the health and human service community.
- Conducts comprehensive biopsychosocial assessment independently or as member of an interdisciplinary team.
- Develops, implements, and monitors person-centered plans of care in accordance with established guidelines and standards.
- Evaluates the effectiveness and benefits of the services provided to participants.
- Provides motivational interviewing and coaching to help participants and caregivers achieve optimal social and health outcomes and goals important to them for quality of life.
- Provides advocacy and facilitates formal and informal care and services through external sources.
- Maintains current, accurate, and HIPAA-compliant participant files and program records.
- Travels throughout Berrien, Cass, and Van Buren Counties for in-home assessments.
REQUIREMENTS:
- Current Michigan Licensure as a Social worker (LBSW, LMSW, LLMSW, LLBSW) OR Registered Nurse (RN)
- Computer literacy in routine office applications is required. (Microsoft Office Suite)
- General knowledge of long-term care, special populations, person-centered thinking, and community resources preferred.
