Description
Care Manager RN at Providence St. Joseph Medical Center in Burbank, CA. This position is full‑time and will work 8‑hour day shifts (Sunday – Thursday). Responsibilities
Collaborate with patients, caregivers, nurses, social workers, physicians, payers, support staff and other practitioners to provide case management and care coordination.
Facilitate communication and care coordination through the continuum of care and effective transitional care management.
Address the patient’s right to self‑determination and consider social determinants of health and care complexities.
Support the organization’s mission of patient‑focused, whole‑person care built on understanding, commitment and mutual respect.
Required Qualifications
Bachelor’s degree or equivalent education/experience.
California Registered Nurse license upon hire.
2 years experience in a healthcare related field (acute, ambulatory, post‑acute, etc.).
2 years experience in case management or successful completion of the Transitions in Practice (TIP) program for Care Manager. TIP candidates must have experience in the same nursing unit in which the CM position is available.
Preferred Qualifications
Bachelor’s degree in Nursing.
Master’s degree in a healthcare related field.
Certification in Case Management (ACM or CCM) upon hire.

Compensation
Pay Range: $56.12 – $84.75 per hour (additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives or bonus opportunities).
