Care Manager RN – Providence St. Joseph Medical Center, Burbank, CA.
This full‑time, 8‑hour day‑shift position (Sunday‑Thursday) serves a Magnet® recognized hospital that is one of the best regional hospitals in 11 types of care by U.S. News & World Report. Case Management is a collaborative practice that includes patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. The case management process facilitates communication and care coordination along a continuum through effective transitional care management, recognizing the patient’s right to self‑determination as well as the social determinants of health and complexities of care.

Responsibilities
The role involves providing case management services, coordinating care plans, and collaborating with interdisciplinary teams to ensure seamless patient transitions and optimal outcomes.
Required Qualifications
Bachelor’s Degree, or equivalent education/experience.
California Registered Nurse License upon hire.
2 years experience in a healthcare related field (Acute, Ambulatory, Post‑Acute, etc.).
2 years experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Manager. TIP candidates must have experience in the same type of nursing unit as the CM position.
Preferred Qualifications
Bachelor’s Degree in Nursing.
Master’s Degree in a healthcare related field.
Certification in Case Management (ACM or CCM) upon hire.
