Description The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple co-morbidities. This position works in coordination with multiple PCPs and serves as the central link to an identified population of patients.

Coordinate with multiple primary care providers (PCPs) and act as the central link to an identified patient population.
Required Qualifications Graduation from an approved, accredited school of Nursing.
Washington Registered Nurse License prior to hire.
Two years of work experience in an RN role.
Experience working with medications and IV solutions understanding purpose, administration, side effects, and contraindications; administers according to established standards and provider orders.
Experience managing complex patients in an ambulatory setting.
Preferred Qualifications Bachelors Degree in Nursing.
