Scope/Position Summary
The Clinical Care Manager functions within the multidisciplinary team to facilitate an assigned group of patients through the health care system with the primary focus on efficient, effective, compassionate inpatient care and a safe transition to the next level of care. In some areas, this may include pre‑admission to post‑discharge coordination of care. The Clinical Case Manager monitors the patient care process and physiological needs; documents, communicates, and collects data to evaluate and improve timeliness and appropriateness of care. The Clinical Case Manager collaborates with physicians, nurses, and other healthcare workers to provide comprehensive case management assessment, treatment, and follow‑up evaluation for patients and to communicate the plan of care. The Clinical Case Manager maintains an awareness of state and national healthcare trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization.

Responsibilities

Facilitate efficient and compassionate inpatient care and safe transitions.

Monitor patient care processes and physiological needs.

Document, communicate, and collect data to evaluate and improve timeliness and appropriateness of care.

Collaborate with physicians, nurses, and other healthcare workers for comprehensive case management assessment, treatment, and follow‑up evaluation.

Communicate plan of care and maintain awareness of healthcare trends.

Proactively respond with initiatives to enhance care provision and resource utilization.

Minimum Qualifications

RN license required; BSN preferred or Licensed Master’s in Social Work with prior case management and/or discharge planning experience.

2 years of clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations.

Prior case management experience strongly preferred.

Must understand the health care continuum and possess strong communication and interpersonal skills.

Ability to work with various computer applications.

Final candidates are subject to successful completion of a background check. A drug screen or physical may be required during the post‑offer process.
