Your Goal
The primary purpose of this position is to facilitate coordination of care and care management for all Greater Family Health third‑party managed care patients.
How You’ll Help Patients
Complete care management screenings and risk assessments for applicable third‑party managed care patients.
Coordinate visits for applicable third‑party managed care patients with their PCP for annual wellness evaluations, chronic care management, transitions of care or other required programs.
Work directly with patients/family members to develop and implement a comprehensive care plan for applicable third‑party managed care patients, addressing medical and behavioral health needs, medication management and social determinants of health.
Provide continuous monthly patient monitoring to assess the efficacy of the care plan in applicable care management programs.
Provide transition of care coordination for hospitalized patients of Greater Family Health in applicable third‑party managed care programs.
Provide patient education regarding labs, procedures and disease processes and any other health education information deemed necessary.
Report to all scheduled work sites and shifts on time and fully prepared; location of work sites and shifts will change at the supervisor’s discretion.
Engage in all offered training, consultations and supervision made available; respond professionally to directives and feedback from supervisors and coworkers.
Perform other duties as assigned.

Qualifications
Illinois licensed Registered Nurse (RN) required.
Bachelor of Science in Nursing (BSN) required.
Active and current BLS/CPR certification.
Bilingual in Spanish is highly preferred.
2–3 years of hospital or outpatient clinical experience highly preferred.
Experience working with pediatric and adult populations highly preferred.
