Role Description
This is a full-time on-site role for a Care Manager/RN located in Holland, MI. The Care Manager will be responsible for providing comprehensive case management services, coordinating care plans for patients, and collaborating with healthcare providers to ensure optimal patient outcomes. The role will involve hands‑on patient care, conducting assessments, developing individualized care plans, and facilitating access to necessary medical and social services. Training others in care management practices may also be required.
Qualifications
Licensed RN in the State of Michigan required
3‑5 years of acute care/med‑surg experience
Care management background preferred
