Job Summary
Talent Software Services is searching for a Care Manager, RN for a 2‑month contract in Wilmington, DE, with potential for a long‑term extension. The role is full‑time, community‑based, and requires frequent travel within Delaware. The Care Manager will coordinate members' care needs across services and community supports, visiting homes, nursing facilities, and other settings.
Primary Responsibilities
Travel to members' homes, nursing facilities, and other settings to conduct face‑to‑face need assessments and follow‑up telephone contacts.
Assess, plan, coordinate, implement and evaluate care for eligible members with chronic and complex health, social service, and custodial needs.
Coordinate care across the continuum of services, ensuring safe, least‑restrictive, and cost‑effective care.
Facilitate authorization, coordination, continuity, and appropriateness of care and services in community or HCBS.
Facilitate transitions to alternate care settings such as hospital to home, nursing facility to community, using an integrated care team.
Educate members or caregivers about health needs, benefits, resources and services, including long‑term care options.
Assist members in achieving care plan goals and finding optimal solutions.
Develop care plans in collaboration with members or caregivers to meet specific needs.
Identify resources and facilitate referrals to specialty programs such as disease/chronic condition management, behavioral health, and complex case management.
Collaborate with members’ health care and service delivery team, including the DSHP Plus LTSS Member Advocate, ICT, and discharge planners.
Assist members in developing and implementing backup plans for gaps in provider coverage.
Ensure that approved support services are provided as outlined in the plan of care.
Evaluate the effectiveness of the service plan and revise as needed according to policy and state contractual requirements.
Help members overcome obstacles by connecting to community resources and formulating action plans.
Document all case management services and interventions in the electronic health record.
Adhere to all company, state and federal privacy practices (HIPAA) and quality performance standards.
Perform additional duties as assigned.
Qualifications
Bachelor’s degree in Social Work or related field and 3 years of experience in long‑term care, home health, hospice, public health, or assisted living.
Master’s degree in Social Work or related field and 1 year of experience in the same areas.
Registered Nurse or Licensed Practical Nurse and 2 years of experience in long‑term care, home health, hospice, public health, or assisted living.
A high school degree or equivalent and three years of case management experience with the aged or populations with behavioral health, developmental disabilities, or HIV/AIDS.
Preferred
One year of home clinical or case management experience.
Certified Case Manager (CCM), Licensed Bachelors Social Worker (LBSW), Licensed Master's Social Worker (LMSW), Licensed Clinical Social Worker (LCSW).
Experience with HIV/AIDS, behavioral health, developmental disabilities populations.
Medicare and Medicaid experience, managed‑care experience, flexible hours, PC‑based word processing and database documentation skills.
Reliable transportation and ability to meet regulatory deadlines.
Proficiency with electronic documentation systems, budgeting, and cost neutrality.
