Sutter Health is looking for a professional to join their team. The position involves coordinating utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation. The role focuses on providing coordinated and integrated care to prevent unnecessary admissions or readmissions. It also requires discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department.
The ideal candidate will have a graduate degree from an accredited school of nursing, be a Registered Nurse of California, and possess Basic Life Support Healthcare Provider certification. They should have at least two years of recent relevant experience and demonstrate a broad knowledge base of health care delivery and case management within a managed care environment.
Key skills include comprehensive knowledge of Utilization Review, levels of care, and observation status, as well as working knowledge of laws, regulations, and professional standards affecting case management practice in an integrated delivery system. The candidate must also effectively communicate with patients, families, caretakers, physicians, nurses, and other ancillary partners, and be able to work independently and exercise sound judgment in interactions with physicians, payers, and patients and their families.
The job shift is days, with a per diem/casual schedule, variable hours, and rotating weekends. Benefits are not provided, but the position is union-represented and non-exempt. The pay range is $70.41 to $93.73 per hour, with compensation taking into account several factors including experience, education, skills, and departmental needs.
