Responsibilities The Care Manager monitors and documents the effectiveness of treatment, coordinates the admission process, and determines appropriate post-discharge services with the interdisciplinary team (IDT).
Coordinates and updates the Care Plan. Attends meetings if requested by the guest or deemed appropriate by IDT.
Participate in daily IDT meetings and provide clinical reviews as necessary.
Complete assigned sections of the Discharge Plan Instructions and review clinical post-discharge instructions with the guest upon discharge.
Qualifications Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
Must possess a current, unencumbered, active license to practice as an RN or LPN/LVN through the State Board of Nursing.
Must have a minimum of two (2) years’ experience in a hospital, nursing care facility, or other related health care facility.
