$40 USD to $50 USD per shift. The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and other job-related reasons.

Position Summary
The Affiliate Care Manager coordinates and manages the care of residents within their affiliate by overseeing internal and external team efforts to achieve outcomes aligned with organizational performance metrics. These include:
Improved outcomes: Length of stay (LOS), ACO value metrics, quality performance, rehospitalizations, communication, and collaborative care management during the short‑term rehabilitation (STR) stay, skilled benefit utilization, and post‑transition outcomes.
Managed care alignment: Compliance with managed care contract requirements to ensure accurate reimbursement, appropriate LOS, quality performance, and effective communication and coordination during the skilled stay and post‑discharge.
The Care Manager collaborates with internal teams and external providers from admission through 30 days post‑skilled stay to ensure a smooth transition to the next level of care. They ensure that post‑acute providers have the necessary information to continue providing high‑quality care.
For residents not returning home, the Care Manager transitions the patient back to the affiliate’s routine census, promoting success through education and coordination with both the patient and the affiliate team.
Additional responsibilities include data tracking, relationship‑building with external programs, staff education, and coordinating efforts based on external drivers.
Licensure & Experience Requirements
Must hold an active Massachusetts RN or LPN license (RN preferred).
Experience in acute care, long‑term care, and/or short‑term rehabilitation required.
Education
Some college or better in Nursing or related field.
Experience
2‑4 years required.
Licenses & Certifications
Registered Nurse.
