Epiphany Family Services seeks to hire Tailored Care Management Extenders I – to join our growing team in the Charlotte region. The Care Manager Extender I will assist in meeting the needs of the member, create additional workforce capacity, and allow care managers and supervisors to focus on key tasks for assigned members as well as permit them additional time for members with intensive or complex needs.
Responsibilities
Performing general outreach, engagement, and follow-up with members.
Conducting introductory engagement calls to enroll members into TCM services.
Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing.
Sharing information with the care manager and other members of the care team on the member’s circumstances.
Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services.
Support the care manager in assessing and addressing unmet health-related resource needs to include following up on ADT alerts and other urgent TCM needs.
Other duties as assigned.
Qualifications
Is at least 18 years of age.
Has a high school diploma or equivalent.
1. Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist 2. Has one year of paid experience performing the types of functions described in the “Extender Functions” section below.
Individuals who will be able to meet these qualifications, including, but not limited to the following:
Certified Peer Support Specialists; or
Community health workers (CHW), defined as individuals who have completed the NC
Community Health Worker Standardized Core Competency Training (NC CHW SCCT); or
Individuals who served as Community Navigators prior to the implementation of Tailored Plans
Knowledge, Skills, and Abilities:
Person-Centered Thinking/planning
Knowledge of using assessments to develop plans of care.
Knowledge of LOC processes
Knowledge of Medicaid basic, enhanced MHSUD, and waiver benefits plans
Familiarity with or intent to learn the use of Motivational Interviewing techniques.
Strong interpersonal and written/verbal communication skills.
Conflict management and resolution skills
Proficient in Microsoft Office products (such as Word, Excel, Outlook, etc.)
High level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance.
Ability to make prompt, independent decisions based upon relevant facts.
Good organizational skills to prioritize duties and work with minimal levels of onsite supervision to consistently meet deadlines.
