Eliot's Community Service Agency (CSA) serves children and youth (through age 21) and their families who are in need of support in managing and coordinating services. Eliot is excited to align these essential CSA services with the catchment area of our established Community Behavioral Health Centers (CBHCs) to offer a seamless and effective community-based continuum of care.
The ideal candidate for the Care Manager - CSA position is a dedicated and collaborative professional with a strong background in child and family behavioral health, crisis management, and care coordination. They possess excellent communication and organizational skills, demonstrate cultural sensitivity, and are committed to empowering families through individualized care planning, system navigation, and ongoing support.
Responsibilities:
Serve as key contact for the youth and family to the Eliot CSA and its services.
Initiate care planning activities with all members of the family and/or caregiving system.
Conduct a comprehensive home-based assessment in conjunction with other CSA Team members inclusive of the CANS, CRAFTT (for youth 12 and older), SNCD and other tools as determined necessary, which occurs in the youth’s home or another location of the family’s choice.
Initiate planning that includes intake meetings, orient the family and the member to the wraparound process, identify strengths and needs of the child.
Develop and implement a youth and family centered Individualized Care Plan (ICP) in collaboration with the family and collaterals.
Develop an initial safety/crisis plan.
Qualifications:
Bachelor’s degree in human services or related field
Must have a valid driver’s license and reliable transportation
Experience navigating child/family service systems
Experience working with child mental health issues and crisis management
Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase
Schedule: Monday - Friday, 9am-5pm $55,000 - $58,000 USD
