Care Manager / Care Navigator
Join us at Master•Care, Inc. to make a difference by supporting vulnerable older adults. We are searching for a caring, dedicated Care Navigator to join our team of professionals who are genuinely making a difference.

Position Summary
A Care Navigator provides Care Management and Transition services to patients using a “Master Care Plan.” You will deliver Person-Centered Care Navigation to help patients achieve their goals and improve health outcomes. This role requires serving patients in person and remotely within the assigned region.

Duties and Responsibilities

Provide person-centered care management to patients in a non-clinical setting, integrating clinical needs and social determinants to create a comprehensive care plan.

Serve as the primary point of contact for assigned patients.

Complete comprehensive in-person assessments of all assigned patients.

Develop and execute the Master Care Plan, managing expectations and communicating clearly.

Respect and understand patients’ goals, wishes, values, beliefs, and perspectives, empowering them to achieve health improvements.

Conduct in-home and virtual patient appointments as required.

Coordinate seamless communication and care transitions among providers and care team members.

Maintain professional relationships with customers, providers, and community resources.

Work with leadership and care teams to achieve ECM goals and ensure compliance with provider and managed‑care plan standards.

Stay aware of applicable laws, regulations, and privacy compliance, handling PHI appropriately.

Maintain a safe, clean, and respectful work environment.

Education and Qualifications

Degree or equivalent credential in social work, gerontology, geriatric care management, or verified professional experience.

Three or more years of quality assurance, delivery, or management experience in health care, community‑based senior services, or a related environment.

Knowledge of and experience with clinical and non‑clinical services for older adults and individuals with chronic health conditions.

Professional communication skills with patients, families, providers, community resources, and internal teams.

Bilingual or multilingual skills desirable but not required.

Physically able to sit/stand for long periods, navigate stairs, bend, reach, lift/push/pull minimum 10 lbs., and drive frequently.

Benefits

Starting pay: $30‑33 per hour.
