Position Summary We are seeking a compassionate and dedicated Integrated Case Manager to join our ICMS team. This role is a community based position responsible for providing targeted case management and wraparound support to adults with serious mental illness (SMI). The Case Manager works collaboratively with clients, families, and service providers to promote wellness, enhance independence, and reduce hospitalizations.

Program: Integrated Case Management Services (ICMS)
Key Responsibilities Provide client-centered case management services in the community, home, or office setting.
Develop individualized service plans that support recovery goals, promote stability, and address mental health, substance use, medical, housing, and social needs.
Create and update crisis and safety plans with clients and the treatment team.
Coordinate access to a broad range of services, including psychiatric care, therapy, substance use treatment, housing support, and community-based resources.
Assist clients in navigating insurance systems, entitlements, and legal concerns.
Identify high-risk clients and participate in team-based approaches to ensure safe and effective care.
Maintain timely and thorough clinical documentation in compliance with agency, state, and funding requirements.
MINIMUL QUALIFICATIONS: Bachelor’s degree in related field and at least 2 year’s experience.
A valid NJ driver’s license with a satisfactory driving history is required
