Overview Provides direct services to meet the psychosocial needs of patients and their families, to include: middle and older adults at SACC. Provides support to the patient care areas.
Responsibilities Provide psychosocial support and direct services to patients and families in assigned care areas.
Assist with care coordination and discharge planning as part of the patient care team.
Qualifications Bachelor's Degree
One (1) year recent experience.
Preferred Qualifications Discharge planning.
