Responsibilities
Conduct comprehensive assessments to understand consumers' unique needs and preferences.
Craft personalized care plans that empower consumers to reside independently in their home.
Coordinate services and resources, ensuring a seamless care experience.
Monitor progress and adjust care plans to achieve optimal outcomes.
Advocate fiercely for consumer rights and needs, becoming their trusted advocate.
Collaborate with a passionate team of healthcare providers and community partners.
Who You Are / Qualifications Holds a Bachelors degree in Social Work, or a related field.
Has experience in case management, particularly in human services or with diverse older adult populations.
Possesses a strong understanding of community resources and healthcare systems.
Communicates with warmth and clarity, building strong relationships with clients and families.
Is tech-savvy, with experience using electronic health records and related software.
Holds a valid drivers license and has reliable transportation.
Satisfactory background check that includes CORI, SORI, SAMs GSA, Mass Health Exclusion List and OIG check.
