Responsibilities
Primary Role: Focus on the management of high comorbidity/high risk patients with chronic diseases.
Reports to Population Health Manager.
Essential Job Duties:
Complete initial and routine assessments for the purpose of identifying patient care needs
Initiate care planning and implementation of a comprehensive care plan and addresses medical
Assist patient with follow up for preventive health and screening tests
Engage members routinely telephonically and in-person via home/facility visits to coordinate services, community resources, and provide health education
Provide education and assist with follow up appointments how to access appropriate care
Identify early risk factors and targeted condition management interventions as necessary
Create, review and revise Care Plans with a focus on health education and improving health outcomes
Advocate for patient needs and services and provide assistance to members in navigating the healthcare system
Patient care coordinators are liaisons between patients and the healthcare system
Ensure that patients receive the care they need and that they understand their medical condition, medications, and other instructions
Focus on reducing hospital admissions through coordinated care and effective discharge planning with other team members
Collaborate with the member\'s PCP and other care team members to deliver and coordinate quality care and services
Ensure completion of required touch points and care coordination activities and document all activities within the member clinical record and/or population-based software
Qualifications
HS Diploma or equivalent
LPN License with 2 years abulatory care experience OR RN license – unrestricted
BSN preferred
Proficient with Microsoft Office
Proficient with electronic health records
Other skills/qualifications

Able to work as a team player as well as independently
