Job Overview The Care Manager, or “Designated Care Manager,” provides high-quality care to residents in assisted living/long-term care and reminiscence neighborhoods. Responsibilities include delivering hands-on support as per the Individualized Service Plan (ISP), ensuring a safe, home-like environment, and demonstrating Sunrise’s mission to champion quality of life for seniors.
Responsibilities & Qualifications Essential Duties Support our Mission, Principles of Service, and Core Values. Responsibilities include:
Review and document resident information in Daily Log.
Develop and update ISPs.
Monitor designated residents, ensuring their safety and well-being.
Observe and report changes in residents’ conditions.
Participate in daily care meetings.
Respond promptly to call bells and security alerts.
Greet and communicate effectively with residents, families, and team members.
Assist with activities of daily living and promote resident independence and dignity.
Engage residents in life skills and enrichment activities.
Maintain safety, hygiene, and infection control standards.
Additional Responsibilities Ensure safety compliance, assist with housekeeping and laundry, serve meals, and support reminiscence activities. Collaborate with the team to promote a positive environment and adhere to all safety and health regulations.
Qualifications High School diploma/GED (per state regulations)
CPR and First Aid certifications
Minimum age 18
Experience with seniors preferred
Desire to serve and care for seniors
